- skin rash
- urine and/or feces odor
- lack of attention to resident’s personal hygiene
- falls resulting from lack of adequate precautions or assistance
- skin tears
- bruises, contusions, or lesions
- bone fractures
- significant weight loss
- dehydration
- disorientation, depression or isolation
